As soon as your 6th grader turns 11 years old, they are ready to receive the following vaccines: Tdap vaccine protects against tetanus, diphtheria, and pertussis, or whooping cough. Meningococcal conjugate vaccine (MenACWY) helps protect against bacterial meningitis. Both of these vaccines are recommended at age 11 and are required before entering 7th grade. In order to enter into 7th grade, all students must have an updated 3231 immunization form showing proof of a whooping cough booster shot and a meningococcal shot. Your child’s pediatrician or the Cobb Douglas Health Department (http://www.cobbanddouglaspublichealth.com/about-us/locations-hours/) will be able to assist you with the vaccines. Please return the updated 3231 form to the main office. 7th Grade Immunization Requirements: English: http://www.cobbk12.org/aboutccsd/enrollment/IMMUNIZATIONInfo_English_2017.pdf Spanish: http://www.cobbk12.org/aboutccsd/enrollment/IMMUNIZATIONInfo_Spanish_2017.pdf |
